Geriatric care management is the process of planning and coordinating care of the elderly and others with physical and / or mental impairments to meet their long term care needs, improve their quality of life, and maintain their independence for as long as possible. Geriatric care management integrates health, social care and psychological care with other needed services such as: home care services, nutritional services, assistance with activities of daily living, socialization programs etc. It is a preventive, proactive approach to healthcare that reduces the risk of hospitalizations, nursing home admissions and healthcare costs. The population of elderly citizens (above 60yrs) is on the rise with Kerala having a highest average. As a part of providing Geriatric care to the needy elderly population, JMMC&RI has embarked on a mission to provide care at your home.
At JMMC, we now provide services for you and your parents right at your doorstep. Our trained professionals conduct a comprehensive geriatric care assessment of your loved ones and prepare a specific care plan according to their health conditions. Our team would visit your loved one at their home regularly and provide medical help in addition to providing supportive care. In addition to this, we would also provide training to the caretaker in supporting the elder. Our team ensures that our patients are given a continuum of care from the hospital right up to their home. Geriatric Home Care allows the medical team to reach to the patient and the family, positively influencing their care plan.
Our team includes Geriatrician, Trained Doctors, Physiotherapist, Dietician and Nurses. They visit periodically and support the autonomy, choice and safety of older adults at home. The team ensures that our patients enter gracefully into their old age, and prevent hospitalization as far as possible.
