The Department of Reconstructive Microsurgery was formed in 2007 as a separate division of Plastic Surgery when Dr. Prashanth Varkey joined. The primary aim was to create a state of art Reconstructive Microsurgery service along with the already well established Burns, Cleft Lip and Palate Services. In order to develop specific areas in the broad arena of Plastic Surgery, Cleft lip and palate care became a dedicated facility under Charles Pinto Center for Cleft lip, Palate and Craniofacial Anomalies headed by Dr H.S.Adenwalla.

After an initial period of shared services, the division Reconstructive Microsurgery made its humble beginning with a 8 bed ward and also took over the management of Burns. The Burns unit underwent a restructuring in aesthetics and functionality getting it geared up to start work in Tissue Engineering. A make shift facility which functioned as Tissue engineering lab facility started adjacent to the Burns unit with the aim of developing affordable dermal substitutes and progressing to work toward bioengineering of human skin. The Bioengeering lab partnered with the Department of Meat Sciences at Government Veterinary College, Mannuthy to start work on gamma irradiated amniotic membrane as a scaffold for tissue engineering of human skin.

In 2013, our in-patient unit of JISHAM has been relocated to the second floor of Jubilee Hridyalaya. Currently our unit performs over 1100 operations per year with existing facilities.

Our Plastic Surgery team consists of talented plastic surgeons, registered nurses and Operation Theatre staff and our Hyperbaric Oxygen Therapy Technicians. The entire team plays an integral role in providing you with the highest quality care possible. Our Plastic Surgeons have been trained in the best institutions in this country and have complemented their skills through exprience gained by there work in Internationally reputed centres. Plastic Surgery at Jubilee Mission Medical College is separated into two fields:

• Cleft Lip & Palate Surgery
• Reconstructive Microsurgery, Burn care & Hyperbaric Oxygen Therapy.

The Reconstructive surgeons at JISHAM are not part of the Cleft Care team and this allows them dedicated focus on Complex surgeries in trauma, General Plastic Surgery including early tangential excision for Burns which often require long hours of work.

Our nurses are the primary contact for patients having surgery and help coordinate patient care. They are also a key source of emotional support, orienting you and your family to the department, answering questions and helping you manage your care.

A dedicated operation room was organized and an adjacent small area began functioning as the Bioengineering lab. Expertise in planning were obtained from Burn unit at Prince of Wales Hospital, Hong Kong. Their guidance helped us start the gamma iradiated amnion bank.

Jubilee Burn Care unit continues to treat over 500 burns each year and is probably only of the only centers in the region to practice of Tangential excision for deep burns even for extensive burns making it a standard practice for better survival.

The team of doctors are supported by a passionate group of nurses and allied staff who help bring out the best results in this challenging work. This unit progressed and developed in-house gamma irradiated amniotic mebrane sheets, which are used largely for pateints here. To keep update with new developments in Burn care, we introduced newer dressing materials that decreased wound odour, pain and frequency of dressings. We were the first in Kerala state to acquire a Meek Mesher that helps in grafting of large raw areas.

This unit has visitors from other parts of the country and occasional international visitors. Our unit embarked on a prevention and awareness program when Dr Prashanth Varkey obtained the support of Mr.AV Anoop (MD,Cholayil Medimix) in producing the first of its kind Burn Prevention animation film that is being screened in schools to create awareness on prevention of accidental burns. This program highlights the first aid of Burn accident is to pour water.

Our center has become the first Burn Center in INDIA to have a Hyperbaric Oxygen therapy Center in 2011 when our Hopsital established the First Hyperbaric Oxygen Therapy in South India.

• For Referring Physicians

  The Hyperbaric Oxygen Therapy Center functions under the Jubilee Institute of Surgery for Hand Aesthetic & Microsurgery (JISHAM) and the Jubilee Burn Center. Our Hyperbaric Program provides treatment in a standard environment with a monoplace chamber. Hyperbaric oxygen therapy is administered in conjunction with the referring physicians/ surgeons plan of care. The standard of care has been established in accordance with the current guidelines of the Undersea and Hyperbaric Medical Society. Treatment duration is for 90 minutes per day. The patient may lie in a gentle comfortable inclined position inside the monoplace chamber and rest, watch television or movies. The number of treatments is patient specific, determined by the diagnosis and response to therapy. Continuous supervision and monitoring are provided by a highly trained and experienced staff of caring registered nurses.

• Inpatient Treatment

  Patients requiring hospitalization, particularly those with critical illness, will generally be admitted to the Jubilee Hriduyalya (Second floor) which has the inpatient facilities for patients undergoing Hyperbaric Oxygen therapy. Inpatients can choose from options of a common ward to individual private rooms

• Outpatient Treatment

  Treatment will be coordinated by our professional staff in close collaboration with the referring physician. Our first treatment starts at 7:30 AM and the final treatment of the day begins at 4:30 PM to accommodate the busy schedules of our patients.

• Benefits of Hyperbaric Oxygen Therapy

  Patients breathe 100% oxygen in the chamber which is pressurized to two-to-three times normal atmospheric pressure. The physiological effects of treatment, which may be of significant benefit to your patient include:

  Hyperoxygenation:

  The plasma carries as much oxygen as saturated hemoglobin with an arterial oxygen pressure of 1,500 to 2,000 mm Hg. This high concentration facilitates increased diffusion into surrounding tissues from perfused capillaries. This effect will last for several hours and can help in both acute and chronic disease.

  Resolution of post-injury edema:

  Vasoconstriction is an immediate reflex response to the hyperoxygenation. Because the hyperoxia is so extreme, the decreased arterial inflow is not associated with hypoxia. There is however, an increase in net venous outflow from congested capillary beds. This change in flow facilitates the resolution of post injury edema, which can often obviate the need for fasciotomy in moderate compartment syndrome and facilitate wound healing.

  Neovascularization:

  Enrichment of ischemic chronic wounds facilitates macrophage and fibroblast function, with improved angiogenesis and fibroplasia. This effect is the basis of treatment of diabetic ulcers, radiation damaged tissue and refractory osteomyelitis.

  Enhanced Immune function:

  High concentrations of oxygen enhances white blood cell phagocytosis and oxidative cell kill, as well as inhibiting and inactivating C. perfringens toxins.

Our Knowledge of regenerative medicine and tissue engineered skin substitutes are utilized to develop artificial skin that completely replicate normal uninjured skin. Natural biopolymers such as collagen from amniotic membrane are being investigated as potential sources of biomaterial to which cells can attach. Our first generation dermal scaffolds development has drawbacks in terms of mechanical strength and degradation properties. We are working along with other groups that have developed synthetic degradable gels and nano spun films as a way to deliver cells and/or molecules in situ, the so-called smart matrix technology. Our work in this unit has won us a research grant from the ER & IPR directorate at DRDO. This project is in collaboration with the Department of Meat Sciences, Government Veterinary College, Thrissur
